To apply, pick up an application at the LHA office, located at 101 Hamilton Heights, Lebanon, Kentucky, 40033, from 8:30 am to 12:00 pm and 1:00 pm to 4:30 pm, ET.

Once the paper application has been completed, it must be dropped off at above address, during office hours.

This waiting list has the following preference: Disabled.

Selected applicants will be placed on the waiting list by date and time, by order of preference.
