There were two ways to apply during the opening period:

  1. Complete the online application.
  2. Contact the MHRA to request a paper application.

Please note: There was an option to complete an online application, but the MHRA had technical difficulties, and removed the online application.




Once the paper application has been completed, it must have been:

  1. Mailed the MHRA office.
  2. Dropped off to the MHRA.
  3. Faxed to the MHRA.
  4. Emailed to the MHRA.

There are no preferences.


Applicants were placed on the waiting list by date and time the application was received.
