To apply, download and print the application online. Click the link titled "Application for Assistance."

The following documentation is required when submitting the application: Birth certificates and Social Security Cards for all members of the household, and photo ID of all adult members of the household.

This waiting list has the following preferences: Victims of domestic violence, veteran, working family

Selected applicants will be placed on the waiting list by date and time the application is received, by order of preferences.
