Visit the PHA to pick up an application, located at 156 North Main St., Pleasantville, NJ 08232, during normal office hours Monday-Friday.2. Request to have an application mailed to you by calling 609-646-3023.3. Download the application here, then print and complete,Once the application has been completed, it can be mailed or hand delivered to the address listed above.
