Once the pre-application has been completed, it must be hand delivered to the address listed above, from 8:00 am until 12:00 pm and from 1:00 pm until 4:30 pm, Monday, Wednesday-Friday.
Be sure to include these documents with the pre-application: Social Security Cards and birth certificates for all household members, final divorce decree (if applicable), marriage certificate (if applicable), current landlords name and complete mailing address, Social Security/SSI award letter (if applicable), child support check stubs (if applicable), unemployment stubs (if applicable), veteran award letter (if applicable), and all proof of income.
