Visit the HACB to pick up an application, located at 612 Ray St., P.O. Box 666, Brush, CO 80723, during normal office hours.2. Request an application be mailed to you by calling 970-842-5046 during normal office hours.Once the application has been completed, it can be mailed or hand delivered to the address listed above.
