This waiting list is open indefinitely.There are two ways to apply:1. Visit the MMHA in person to pick up an application, located at 4580 OH-376, McConnelsville, OH 43756, during normal office hours.2. Request an application be mailed to you by calling (740) 962-4930 during normal office hours.Once the application has been completed, it must be hand delivered to the address listed above during normal office hours.
