There are three ways to apply:
Once the application has been completed, it must be hand delivered to the OCHA office at the address above.
The following documents are required with the application: Social Security Card, birth certificate, and income/assets of all members of the household; and photo ID of all 18+ members of the household. Copies can be made at the OCHA office.
This waiting list has the following preference: Veteran.
Selected applicants will be placed on the waiting list by date and time the application is received, by order of preferences.
