The DSHA last accepted applications for this waiting list from the earliest confirmed date of March 9, 2022, until March 18, 2022. There is no notice of when this waiting list will reopen.

Once the paper application was completed, it was emailed, mailed or faxed to the DSHA. Any applications received after March 18, 2022 will be returned.
