Visit the housing authority office to pick up an application, located at 240 East Center St., Provo, UT 84606, Monday-Thursday between the hours of 7:30am and 5:30pm.3. Complete an application online here.Once the paper applications have been completed, they can be mailed or hand delivered to the address listed above.
