Visit the housing authority to pick up an application, located at 11 Old Meadow Rd., St. Johnsville, NY 13452, Monday-Thursday 9:00am-4:00pm, and Friday 9:00am-1:00pm.Once the application has been completed, it can be mailed or hand delivered to the address listed above.
