  1. Visit the CHA to pick up an application, located at 825 McAdoo St., Clinton, TN 37716, from 8:00 am until 4:30 pm EDT (closed from 12:00 pm until 1:00 pm), Monday-Friday.
  2. Request to have an application mailed to you by calling (865) 457-9692, from 8:00 am until 4:30 pm (closed from 12:00 pm until 1:00 pm), Monday-Friday.
